Mine had bad caps on it, had to change them in late 2003, so they only lasted about 3 years.
Seems to use PSU 5V for Vcore and PSU 12V for everything else, with a 3.3V regulator circuit in the corner near the SDRAM slots. Doesn't use PSU 3.3V at all.
